You have declined participation in this registry.
A research registry is a collection of individuals interested in learning about research studies that may be of interest to them. We are inviting you to join in the Human Engineering Research Laboratories (HERL) Assistive Technology Registry because you are interested in participating in our current or future research studies.
You are being asked to take part in the HERL Registry because you are 18 years of age or older and use any type of assistive technology (e.g. wheelchair, scooter, prosthesis, cane, etc.).
If you agree to be included in the HERL Registry, you will complete a brief questionnaire that will ask:
You will also be giving your permission for the staff members of the HERL Registry to contact you about current or future research studies in which you may be interested. You are not obligated to participate in any research studies about which you are contacted. If you decide to participate in a research study you are contacted about, we may obtain information already on file in the Registry for use in that study.
Occasionally, researchers from outside of HERL may request permission to use the HERL Registry to identify subjects for their research study. Each request will first be reviewed by the Directors of HERL and, if approved, you will be contacted by Registry staff and provided with information about the study with contact information if you are interested. We may also review the registry data in order to examine trends in AT use.
Several procedures have been put in place to protect the confidentiality of your research records. Access to your data is limited to the investigators associated with the Registry, the University Center for Social & Urban Research (UCSUR) who maintains the online database and University of Pittsburgh Research Conduct and Compliance Office, who may access your identifiable information in order to monitor the registry.
There are few risks associated with participating in the HERL Registry. As private information is collected about you, there is a risk to your privacy and confidentiality. The research staff and UCSUR will take every precaution to protect your identity and the confidentiality of the information collected about you. You will not directly benefit from being in the HERL Registry, however, the information collected may help the research team evaluate and/or improve assistive technology that may possibly help you and other individuals who benefit from these devices in the future.
Your participation in this HERL Registry is voluntary. You do not have to take part and if you do not want to be a part of the HERL Registry, there is no penalty or loss of rights. You may stop being a part of the HERL Registry at any time. Being a part of the HERL Registry, or choosing not to be a part of it, will have no impact on your relationship with the University of Pittsburgh or University of Pittsburgh Medical Center.
You will not be paid for participating in the HERL Registry.
If you have any questions about this study, please contact the Clinical Coordinators at the Human Engineering Research Laboratories at (412) 822-3700 or email@example.com.
